Dry eye

Dry eye

Dry eye is a common ocular condition characterized by insufficient tear production or rapid tear evaporation, leading to discomfort and potential vision problems. This chronic condition affects millions worldwide and can significantly impact quality of life if left untreated.

Key points:

  • Causes: Often results from meibomian gland dysfunction (MGD), affecting up to 86% of dry eye cases
  • Symptoms: Include burning, stinging, sticky tears, light sensitivity, and blurred vision
  • Diagnosis: Typically involves a comprehensive eye exam and tear film evaluation
  • Treatment: Range from over-the-counter eye drops to advanced therapies like Lumenis IPL
  • Prevention: Lifestyle changes such as proper hydration and digital screen breaks can help

In-depth explanation

Dry eye occurs when the eyes fail to maintain a healthy layer of tears to lubricate the ocular surface. This can happen due to insufficient tear production or increased tear evaporation. The tear film consists of three layers: oil (lipid), water (aqueous), and mucin. Disruption in any of these layers can lead to dry eye symptoms.

Meibomian gland dysfunction (MGD) is the leading cause of evaporative dry eye. These glands, located in the eyelids, produce the oil layer of the tear film. When they malfunction, tears evaporate too quickly, leaving the eye’s surface unprotected.

Common Symptoms

  1. Stinging or burning sensation
  2. Feeling of grittiness or foreign body in the eye
  3. Excessive tearing (paradoxically)
  4. Blurred vision, especially when reading or using digital devices
  5. Light sensitivity
  6. Red, irritated eyes
  7. Difficulty wearing contact lenses

Risk factors

  • Age (more common in older adults)
  • Gender (women are more susceptible)
  • Certain medical conditions (e.g., rheumatoid arthritis, diabetes)
  • Prolonged digital screen use
  • Environmental factors (wind, dry climates)
  • Certain medications

Treatment options

Treatment for dry eye typically involves a multi-faceted approach:

  1. Artificial tears: Over-the-counter lubricating eye drops
  2. Lifestyle changes: Increasing humidity, taking screen breaks, improving diet
  3. Prescription medications: Such as Restasis or Xiidra
  4. In-office procedures: Including LipiFlow, BlephEx, or Lumenis IPL
  5. Punctal plugs: Small devices inserted into tear ducts to prevent drainage
  6. Advanced therapies: Such as autologous serum eye drops

Prevention and management

  • Stay hydrated by drinking plenty of water
  • Use a humidifier in dry environments
  • Practice good eyelid hygiene
  • Wear protective eyewear in windy or dry conditions
  • Follow the 20-20-20 rule when using digital devices
  • Consume a diet rich in omega-3 fatty acids
